Hi,
My first thoughts are to look for the brand name and possible model # of your unit. Go directly to their website and look for replacement parts.

The difference between success and failure is who gives up first!
That is called a turbine vent if I'm viewing it correctly.

The other end looks like the chimney with part of the flashing still attached. You may end up getting the whole chimney replaced. It might be safer that way anyways. You can then purchase the entire system from 1 manufacturer and thus be ensured that it will work together as a system.

That's just the chimney vent cap. It's basically to keep the weather and nesting animals out of your chimney.
The type/model of your fireplace may determine what you need. You should check with your fireplace manufacturer to see what kind they recommend. If they are unavailable, just call a local fireplace dealer and tell them what you're looking for.
