I have cathedral ceilings and the shower arm/head are at least a foot above the tub and the surround. We're not giants and I feel like I need to stand on my tip toes to swivel the shower head.

When I installed a new shower I intentionally raised my shower head above, so that I would not have to cut the new surround. Then I added a longer down pipe and a different style (Longer Type) shower head. Works Great!

It was too low and I had to bend over to wet my hair.
I bought an extension for the shower arm that swiveled around and I could move it to make it higher. Let me see if I have a picture.

Some require unscrewing the old pipe from the wall and some will screw onto the existing pipe.

The question was how to lower the shower head.
Yes it can be done. You would need to have access from the back side of the shower wall. Shut off your water, cut pipe to desired height, drill new hole in shower surround, reattach everything....then as Greg said, find a decent patch method for the hole left behind.
