Those piers should have a solid 2x6 wood cap on top of them. Then wedges driven in from opposite sides.
If cap is not installed, weight is not evenly distributed which can result in failure of the block.

He quoted you for aprox 1282 sq ft of sheeting assuming sheets are 4x8.
You say your house is 2016 sq ft. So he is not over on sheeting.
Stick with plywood tongue and groove.
Glad to see he is going to screw and glue.
That is a good sign.
Make sure plywood does not have allot of plug holes.

Tar will eat the rubber EPDM/TPO roofing.
Find a supplier or contractor that has the roof jack or find a another installer.
Do you need a new furnace and wtr heater?
Have a electrician do a load calc--may be you can go elec on wtr heater.

They can install foam under the TPO/EPDM which helps.
TPO/EPDM does deaden some of the noise.

You are correct! If you can afford it install either EPDM or TPO rubber roof membrane. Check with you local energy folks at PUD. Depending on your income they come out and put it on for free. Pull the mush board and install 3/4 plywood, use grade aproved for vinyl install.
If your fasteners don not hold in mush board they will start popping and come through vinyl

Let me add that the rafter jig may be off or the camber of the rafters may be off.
You may want to contact your State SAA. They handle consumer complaints on MFG homes.

When you say see the ceiling joist I assume you mean a slight rise every four feet.
